I’m Board Certified Plastic Surgeon, Dr. Matthew Schulman, and let’s talk a little about Breast Augmentation with breast implants.

Breast Augmentation is consistently the most commonly performed cosmetic procedure.

During a breast augmentation, breast volume is increased by placing a breast implant.

With breast implants, there are several choices that you as a patient needs to make together with your surgeon. This includes implant material, implant position, implant volume, and incision placement.

Implants come in a variety of shapes, sizes, and material.

The silicone gel that is used today is very different from the silicone that was used many years ago. Today’s gel is a cohesive gel, called “gummy bear”. This means that if I were to cut an implant open, the gel would not leak out. This is very different that the silicone oil that filled implants from the the 80s and 90s. So the advantage of today’s gel is that the risk of leaking and spreading is minimized, and it looks and feels more natural.

A saline implant is filled with salt water. Some people feel more comfortable with a saline filled implant because if it were to break of leak inside the body, they think it is safer because it is only salt water. This is reasonable position. The disadvantage of saline filled breast implants is that they tend to look and feel a little less natural than their saline counterparts.

Regarding the implant volume, this is another decision best made with your surgeon. The size you choose is partially based on what you want – meaning the look you are going for. However, the implant size also needs to be based on you body shape and size. It is critical to choose an implant that “fits” your bodies dimensions. Choosing an implant that is too big for your body may cause significant complications in the future.

This is why I always take specific measurements when helping my patients choose implants so that I can guide them and prevent them from picking implants that are too big.

So implant size is a based on combination of what you want, and what you can have.

There are also a few options when it comes to incision placement. It is the incision that determines where the scar is. Implants can be placed through the underarm, around the areola, or in the breast fold. Each approach carries different risks in terms of infection, malposition of the implant, sensation changes, and capsular contracture (or scarring around the implant). In general, based on significant data, placing the incision in the breast crease carries the lowest risk of complications. But again, this is an individualized decision and needs to be discussed with your board certified plastic surgeon.

Breast augmentation

Breast augmentation and augmentation mammoplasty (colloquially known as a “boob job”) are plastic surgery terms for the breast-implant and the fat-graft mammoplasty approaches used to increase the size, change the shape, and alter the texture of the breasts of a woman. As a primary reconstruction, augmentation mammoplasty is applied to effect a post–mastectomy breast reconstruction, the repair of the chest wound consequent to the removal of a cancerous breast; to correct congenital defects of the breast(s); and to correct congenital defects of the chest wall. As an elective, cosmetic surgery, primary augmentation changes the aesthetics – of size, shape, and texture – of healthy breasts.
